Terms and conditions

I agree:

  1. I have read and understand the ResHealth Terms of Use (‘terms of use’) and agree they form a legally binding agreement (The 'Agreement') between myself and the State of Queensland acting through RSHQ;
  2. I consent to the collection, use and disclosure of my personal and medical information in accordance with the Privacy statement contained in the terms of use;
  3. I must access and use the ResHealth Portal (‘ResHealth’) and information contained therein (‘confidential information') strictly in accordance with the terms of use;
  4. I will not disclose confidential information, except as permitted or required by law.

I understand that, consistent with the terms of use:

  1. My activities in ResHealth are tracked and logged against my login credentials;
  2. My access and use of ResHealth is monitored, including for identification of breaches of the Agreement and inappropriate and unlawful activity;
  3. If I breach the terms of use, RSHQ may take action against me, including if any of my access or use of ResHealth, or my dealing with confidential information is inappropriate or unlawful;
  4. My personal information, including my medical information, collected as a result of my access and use of ResHealth is dealt with in accordance with:
    a. the terms of use; and
    b. the Coal Mining Safety and Health Act 1999 (Qld), and the Coal Mining Safety and Health Regulation 2017 (Qld);
  5. Ticking this checkbox is a condition of me accessing and using ResHealth.
